$60 MONTHLY Included:

1 Ultimate Service per month

5% off Retail Products (excludes items already discounted)

Pay $60 for any addition Ultimate Services while a member ($90 for Hydrafacial)

Ultimate Services:

• Microdermabrasion
• One Hour Facial
• Obagi Blue Peel Radiance
• Lipo B Injections (1 a week x4 weeks)
• Vitamin IV Infusion Treatment
• Dermaplaning

⃰ Upgrade to a HydraFacial Treatment for $30 (counts as your one ultimate service)

Membership Details:

• Clients choose one service per month (see above) included in membership price.
• Clients are entitled to 5% off all retail products while a member (excludes items already discounted).
• Members pay only $60 for any additional ultimate services whiles a member ($90 for hydrafacial)
• 6 month minimum contract then converts to a monthly membership thereafter.
• 24hrs notice must be given for cancelation of appointment or a $25 no-show fee is applied.
• Credit Card will be charged monthly on the day you enroll in the membership for 6 months then will continue to bill on a monthly basis unless we receive written notice of cancelation 15 days prior to the next billing date. You must also notify the spa of any changes of address.
• Members are required to notify the spa of any changes to credit card on file prior to the next billing cycle or you will be responsible for any CC processing charges we incur.
• Memberships cancelled before the end of the 6 month contract will result in a $100 cancellation fee.
• Services are for the Member only and are non transferable.
• Services do not rollover, you must use your service or you forfeit the service that month.
• You must agree to follow spa rules & regulations. We have the right to refuse or discontinue service for any reason. La Vie
• MedSpa reserves the right to change spa rules, regulations, and pricing at any time upon providing reasonable notice.
• No refunds will be given for unused services.
